Who needs a visa?

The majority of countries require visas to come to the UK (even for a visit). You can check if you require visa by going to http://www.ukba.homeoffice.gov.uk/policyandlaw/immigrationlaw/immigrationrules/appendix1.

Even non-visa nationals require entry clearance to enter in certain categories, such as a spouse.

How do I apply?

The type of form that you need to complete depends on your application and information can be found on http://www.ukvisas.gov.uk/en/howtoapply.

However, although an entry clearance application may appear straightforward from the UK Border Agency website, the forms are detailed and must be filled out accurately. If you fail to complete a form accurately (even if it is an innocent mistake), it is possible that you will be considered to have made an application using deception which gives grounds for any subsequent entry clearance application to be refused.

In addition to the forms themselves the documentary requirements are detailed and must be adhered to, otherwise an application can fail simply due to a genuine error.
